I have been terraforming a lot of planets in my recent game, and I have noticed that planets do not appear to update their graphic (in the colony view) to the new terrain type once completed. I didn't recall noticing this earlier in the game, but it may be that I just overlooked it. I found that if you click end turn, it is still not updated on the following turns in the colony view. When you exit to the menu and load the save, the colony's graphic is updated to the new terrain. Let me know if you need anything to help troubleshoot this.
